SriniVas R. Sadda, MD, while narrating an encounter with a patient during early training, comments on the pathophysiology of diabetic macular edema and how in that era the unrelenting progress of this disease could only be partially treated by lasers, the therapy of the day.

The intervening decades have brought not only new technologies that allow for refinement of screening protocols but also new treatments, such as anti–vascular endothelial growth factor therapy, that offer greater response than former techniques.
